In Diablo 4, Unique items are arguably the highest-concentrated doses of dopamine you can get a taste of. Uniques are so powerful that they often completely change the way builds feel, and sometimes even open up some entirely new playstyles.
However not every Unique item is created equally. Even though most Uniques become available as a drop once you enter World Tier 3 (Nightmare difficulty), there are some that cannot be seen until you reach level 85 in the Torment difficulty.
There are Unique items that the Diablo 4 developers classify as 'Super Rare Uniques'. There are a total of six of them, and they have an incredibly low chance to drop - we're talking one in a million.
In this article, we'll share everything we know about these rarest Diablo 4 Unique items - where they can drop, and how often. We'll also give a brief overview of each of these items and why are they so powerful.

How to Get Diablo 4 'Super Rare Unique' Items
Everything we know about these "Super Rare Uniques" comes from Lead Class Designer for Diablo 4 Adam Jackson, who shared on Twitter:
Hello! I wanted to clear up some details on the rarest Unique items in Diablo 4.
1. They can drop from level 85+ enemies;
2. You get them anywhere you can get a regular Unique, and they always drop at 820 Item Power;
3. We currently have 6 of them in the game;
4. They're really rare!
As to whether or not any specific target farming methods exist, Jackson clarified:
Whenever you get a Unique there is a chance for that Unique to instead be one of these. Therefore, the best way to farm these is to do content that gives you the most Uniques per x period of time.
As such, it seems that in Diablo 4 there are no guaranteed ways to target farm these Super Rare Unique items. To get these items, players must be at least Level 85 in Nightmare difficulty (World Tier 4). Players must simply get lucky while completing content that rewards Uniques, like Helltide Mystery Chests, Nightmare Dungeons, and other endgame activities.
We don't know the exact drop rate of these items, but it must be astronomically low. The first Harlequin Crest ('Shako') was found on June 11, 2023, around a week and a half after the game's early access launch – and the first drop of The Grandfather came June 28, 2023, nearly a month after release.

The Grandfather
This two-handed sword has a long-running history within the Diablo series. It first appeared in the original Diablo, where it was considered one of the most powerful Unique weapons in the game. The Grandfather went on to appear in Diablo 2, Diablo 3 - and in Diablo 4 its power is similarly monstrous.
Not only does it provides huge Base Damage, but it also increases your raw damage output, Maximum Life, Critical Strike Damage, and general stats by incredible amounts. The Grandfather also includes an unusual property that allows all of its other affixes to exceed their normal roll ranges.

Doombringer
With a name like Doombringer, you expect this sword to create huge waves of destructive darkness with every swing - and it delivers. This blade’s Unique aspect is a Lucky Hit trigger, with up to a one in four chance of creating an AOE Shadow damage blast, hitting all nearby enemies and reducing the damage they deal by 20 percent for five seconds.
Doombringer scales very well with Attack Speed, and its other abilities increase your overall damage and Maximum Life, making Doombringer one of the most universal Diablo 4 Super Rare Uniques to slot into any given build.

Andariel's Visage
Living up to the name of one of the lesser evils, Andariel, the Maiden of Anguish, Andariel’s Visage has a lot to offer. It provides massive Armor increase, as well as Attack speed and an added Lifesteal.
The best part of Andariel’s Visage is its Unique Aspect, however, which grants you up to a 20 percent chance to summon a Poison Nova on hit, dealing huge damage over time to all enemies within range. The only unfortunate - but thematic - affix on this helm is the Poison Resistance, which seems like a waste, given how weak Resistances generally are.

Harlequin Crest
While the other Super Rare Uniques are very powerful, and can even be build-defining, the Harlequin’s Crest (often called 'Shako' within the community) is arguably the best item in all of Diablo 4.
Plus four Ranks to all of your current Skills is an absurd bonus, but consider that it also unlocks all of your classes' previously unlearned skills and passives! This is something that literally isn’t possible otherwise by any other means. No matter what class or build you’re running, the Harlequin Crest is Diablo 4’s Holy Grail, and the item by far the most deserving of the ultra-rare status and drop rate.

Ring of Starless Skies
If you run a build where you rely on a Core Skill, which you most likely do, then the Ring of Starless Skies may just be your best in slot. Not only does it provide a huge boost to damage with great Critical Chance and Critical Damage affixes, but it also reduces the Resource cost of each consecutive Core skill.
- 25% Cold Resistance
- 25% Fire Resistance
- +3.2%-6.0% Lucky Hit Chance
- +1.8%-5.0% Crit Chance
- +14%-19% Crit Damage
- +12.5%-19.5% Core Skill Damage
- Aspects:
- Consecutive Core Skills reduce the resource cost of the next Core Skill cast by 8%-12%, to a max 40% reduction.
Melted Heart of Selig
All of the 'Super Rare Uniques' break at least something in the way Diablo 4 is supposed to be played, but Melted Heart of Selig goes on a new level.
It provides a 30 percent boost to your Maximum resource but also makes your Resource pool act as a sponge, absorbing damage for you while you have some available. It’s an interesting amulet that changes how you build your character in a big way. The most obvious synergy comes with the use of the Barrier - it allows you forget about Resource concerns, have your cake and eat it too!
